EMS Provider Certification - Convert to License Form Request
Submit the information below to request a Convert to License form be pushed to your OATH account. Once added, the form will be available for 10 calendar days, after which time it will expire and you must request it again.

Currently certified Colorado EMS providers who are within 6 months of their certification expiration date should use the normal "EMS Provider Certification Renewal" application to apply for license conversion.
